Completion of Form SEP-7 is mandatory for all personnel undergoing separation from the Division. "Separation" is defined to include: voluntary resignation, involuntary termination, mutual agreement, existential reassignment, administrative dissolution, transfer to ████████████████, and any other condition in which a personnel record must be closed. Separation also includes disappearance. Disappearance is not an exemption from this form.
