Section 1: Summary

On or around ████████████, personnel at ████████████████████████████ reported ████████████████████████████████████████████████████████████████████████████████. Initial response teams were ████████████████████████████████████████████████████████. The ████████████████████████ was described by surviving personnel as ████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████████.

At approximately ████ hours, a secondary ███████████ was observed moving toward the populated areas. "I was at my station when I heard ███████████████████████████████████████. I turned around and saw ████████████████████████████████████████████████████████████. I want to be very clear that what I saw was not a person. It was moving like a person moves but it was ████████████████████████████████████. It looked at me. I do not think it had ███████████████████ but it looked at me."
"The thing I keep thinking about is that it recognized me. I had never seen it before but it recognized me. Like it had been ████████████████████████████████████."
INTERVIEWER NOTE: Witness became distressed when asked to elaborate. Witness could not elaborate. Witness could no longer identify what there was to elaborate on. Interview terminated.

WITNESS ████ — DESIGNATION: BETA

"There was nothing unusual. I don't know why I'm here. The shift was completely normal. I went home at 0600 like always. I'm not sure why you're saying I was there for ████████████████. I left at 0600. I always leave at 0600."
NOTE: Witness BETA's access badge logs show no exit event on the date in question. Witness BETA's access badge logs show no entry event either. Witness BETA's access badge logs show nineteen consecutive hours of readings from a location that does not exist within the facility.

INTERVIEWER NOTE: Witness GAMMA's third interview was the most detailed. Witness GAMMA described ████████████████████████████████████████████████████████████████████████ for approximately four hours without pause. The transcript has been classified at the highest available level. The transcriptionist has been reassigned to a department that does not appear on the organizational chart.

WITNESS ████ — DESIGNATION: DELTA

NOTE: Witness DELTA was interviewed sixteen years after The Incident. Witness DELTA maintains they were not present in 2003. The biometric records, psychological profile, and personal effects recovered from the site suggest otherwise. Witness DELTA has been informed of this. Witness DELTA has seen the evidence and continues to deny presence. We are not certain Witness DELTA is lying. We are not certain Witness DELTA is the same person who was there. We are not certain these are different possibilities.

Section 4: Casualties and Personnel Accounting

[REVISED COUNT, 17:00] NOTE: Headcount now complicated by presence of ███████████ personnel who cannot be verified as having ever been employed here. They have identification. The identification is correct. The personnel are incorrect.

Section 5: Recommendations

The following recommendations are submitted by the Incident Review Board. The Board convened on ████████. Three of five Board members were present. The other two were present in a different sense. Recommendations passed by █-█ vote. The vote has been sealed under Protocol W.

■	Immediate reclassification of the site to UNINHABITABLE (CLASSIFICATION █). Site to remain sealed for a minimum of ████ years. margin: "minimum" is optimistic. there is no maximum.
■	Administration of Protocol W to all witnesses, first responders, and ████████████████████. Protocol W should be applied within ████ hours of exposure. margin: Protocol W was applied to seventeen personnel. Fourteen responded as expected. Three responded [REDACTED].
■	Termination of research project ████████████████████████████████████. All materials, findings, and ████████████████████████ to be transferred to ████████████████████████████████████ for containment.
